Offers a support group for teens, ages 12-17.
Thursdays 1 - 2 PM

TOPICS INCLUDE:

Promoting a sense of individual responsibility
Teaching teens about the effects of illegal
drug use
Teaching teens about social/peer pressure
Teaching teens about the dangers of
emerging drugs
Engaging teens in the learning process
Incorporating activities that reinforce
prevention.

Currently, we are operating with one
Community Health Rep. Our "service
area" is Hopland/Ukiah.

The CHR is available, by appointment,
for office of home visits for the
purpose of monitoring (BP/BSL), health
education consultations and case
management issues.

Our CHR transports patients to
medical, dental and behavioral health
appointments on Mondays and
Thursdays.

Transportation requests need to be
made with at least 24 hours advance
notice, schedule permitting.

Cancellation of transportation services
also requires a 24-hour notice.
("Notices" are required for this service
to operate sufficiently and fairly.)

Transportation is scheduled on a first
come, first serve basis.

Rx pick-up and delivery available on
Mondays and Thursdays.
At this time, I would like to thank the
residents of the Hopland Reservation for
their patience, cooperation and flexibility
when scheduling their appointments to
coincide with my transportation schedule.
